Allina Health is seeking a Registered Nurse with wound and ostomy expertise to serve in an outpatient wound clinic setting in Minneapolis, MN. The role requires coordinating care, assessing complex wounds, ostomies, and continence conditions, and documenting plan of care in collaboration with the patient and family.

Tiered experience expectations include minimum 2–5 years in wound care across home health, outpatient clinics, or inpatient units, with current/active WOC certifications.

Qualifications

• RN with wound/ostomy experience in home health, outpatient clinic or inpatient settings.
• Active Wound & Ostomy certifications.
• Strong assessment and care planning skills.
• Ability to coordinate care and document per guidelines.

Responsibilities

• Assess and plan care for patients with wounds/ostomies.
• Collaborate with care team to implement plans and document interventions.
• Monitor patient progress and adjust plans as needed.
• Participate in quality improvement and professional practice.
• Provide education to patients and families.
