Allina Health is seeking a Registered Nurse for outpatient care within Minnesota. The role focuses on coordinating and delivering patient-centered nursing care in a specialized outpatient clinic setting.

The position details include 1.0 FTE (80 hours per two-week pay period), 8-hour day shifts from 7:00am to 3:30pm, and no weekend work. Responsibilities include assessment, planning, implementation and evaluation of patient care, with potential mileage reimbursement for floating to other sites.

Qualifications

• Associate's or Vocational degree graduate of an accredited school of nursing.
• 0 to 2 years RN experience.
• Licensed Registered Nurse - MN Board of Nursing required if working in MN on hire.
• Bachelor's degree in nursing preferred.

Responsibilities

• Assess patient health data and collect relevant information.
• Analyze data to identify nursing diagnoses and plan care.
• Develop and implement individualized care plans with the team.
• Coordinate care with patients, families and health care providers.
• Document care services and outcomes according to guidelines.
