Job summary

Personic Health seeks a Wound Care Nurse (RN or LPN) for an in-home mobile role serving South Chicago and surrounding areas. A reliable vehicle and daily travel to patients' homes and facilities are required.

You will work with a licensed clinical team, perform wound care tasks, ABI assessments, and use Ultramist therapy and wound swabs. Training is provided, with a comprehensive benefits package included.

Qualifications

• Wound care experience required; RN or LPN license in Illinois.
• Ability to travel daily to patient homes and care facilities.
• Knowledge of infection control and safety procedures.
• Strong communication and interpersonal skills.
• Experience with Electronic Medical Records (EMR) systems.
• Ability to perform focused assessments and wound care tasks.
• Capable of capturing digital consents when required.
• Reliable transportation and own vehicle.

Responsibilities

• Perform tasks under the direction of a licensed clinical staff member of a wound care team.
• Utilize specialized knowledge and training based on wound care principles.
• Work under clinical supervision/delegation of a medical provider or licensed nurse.
• Perform duties according to orders, prioritising patient safety and regulatory compliance.
• Conduct ABI assessments.
• Facilitate Ultramist Ultrasound Therapy.
• Carry out wound swabs.
• Travel daily to support home and facility-based care.
