Wound Care on Wheels is seeking a dedicated Mobile Wound Care Licensed Practical Nurse (LPN) to join our growing team. The ideal candidate will provide hands-on wound care services, educate patients, and collaborate with healthcare professionals to ensure optimal healing outcomes.

This role requires strong clinical skills and the ability to work independently in a mobile healthcare setting. Responsibilities include wound assessments, dressing changes, education for patients and caregivers,

Qualifications

• Minimum of 1 year of wound care or home health experience.
• Licensed Practical Nurse (LPN) in Nevada.
• WCC certification preferred but not required.
• Excellent communication and documentation skills.
• Ability to work independently in a mobile care setting.

Responsibilities

• Provide direct wound care under supervision of NP or physician.
• Perform wound assessments and dressing changes; assist debridement.
• Assist with advanced wound care techniques as needed.
• Educate patients and caregivers on care and prevention.
• Document encounters in EMR in compliance with HIPAA.
• Monitor progress and report changes to supervising provider.
• Coordinate care with home health agencies and physicians.
• Travel to patients' homes and facilities within the service area.
