Job summary

Personic Health in Milwaukee, WI seeks a Wound Care Nurse (RN/LPN) for in-home patient visits across homes, assisted living facilities, and other care settings. This full-time, on-the-road role requires you to travel using your own vehicle, with training provided.

Responsibilities include wound management and coordination with the clinical team. You will work under supervision of a licensed professional, perform ABI assessments, Ultramist therapy, wound swabs, and other treatments while ensuring

Qualifications

• Proficiency in wound care
• Basic patient care skills
• Understanding of infection control
• Excellent communication and interpersonal skills
• Ability to care for cognitively impaired clients
• Competence in focused assessment
• Ability to carry out delegated tasks and report appropriately
• Management of wound care materials and supplies
• Reliable transportation and willingness to travel as directed
• Prior experience with EMR systems
• Capture digital consents when required

Responsibilities

• Perform assigned tasks under the direction of a licensed clinical staff member of a wound care team
• Utilize specialized knowledge, training, and skills acquired through our training program based on clinical research and wound management principles
• Work under clinical supervision/delegation of a medical provider or licensed nurse
• Perform duties according to prescribing medical provider orders, focusing on patient safety, and adhering to legal guidelines and regulations
• Conduct ABI assessments
• Facilitate Ultramist Ultrasound Therapy
• Carry out wound swabs
