Job summary

North Valley Specialty Group (NVSG) is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to deliver advanced wound care in the Sacramento region. This field-based role includes a dedicated wound care technician and full administrative support for scheduling, billing, and documentation.

You will travel daily to skilled nursing facilities and homebound patients, perform wound care, assess and plan treatments, document in the EMR, and collaborate with facility staff and families.

Qualifications

• Active, unrestricted CA NP or PA license.
• Current DEA registration or eligibility.
• Minimum 1 year of clinical experience; wound care experience preferred.
• Valid California drivers license and reliable transportation.

Responsibilities

• Perform advanced wound care treatments, including debridement and skin substitute grafts, in SNFs and home settings.
• Evaluate wounds, develop treatment plans, and monitor healing progress.
• Document visits accurately in the EMR.
• Coordinate with wound care technician for supplies and charting.
• Collaborate with facility staff, primary care providers, and families.
• Travel to patient locations throughout the Sacramento region on a structured daily route.
