NIVA Health is seeking an experienced Advanced Wound Care Provider (NP/PA) to deliver high-quality wound care in patients’ homes and local facilities across multiple states. You will guide clinical decision-making, use evidence-based therapies, and drive patient-centered outcomes.

In this mobile role, you will mentor teammates, participate in quality initiatives, and maintain meticulous EMR documentation in a dynamic care environment. Valid license and strong communication skills are essential.

Qualifications

• Active NP/PA license with state credentials.
• Proven advanced wound care experience across diverse patient populations.
• Ability to work autonomously in home and facility settings.
• Excellent clinical decision-making and critical thinking.
