Job summary

North Valley Specialty Group (NVSG) is seeking a Nurse Practitioner or Physician Assistant to provide advanced mobile wound care in SNFs and home settings across Northern California. You’ll work with a wound care technician and administrative support to focus on clinical care.

Travel daily within the Sacramento region, with a 4-day work week and no cap on earnings. Expect patient visits, documentation, and collaboration with families and facility staff.

Qualifications

• Active, unrestricted California NP or PA license (required).
• Current DEA registration (or eligibility).
• Minimum 1 year of clinical experience; wound care experience strongly preferred.
• Debridement and skin substitute graft experience a plus.
• Experience with SNF or home-based care a plus.
• Comfortable working independently in a mobile, field-based environment.

Responsibilities

• Perform advanced wound care treatments, including debridement and application of skin substitute grafts, for patients in SNFs and home settings.
• Evaluate wounds, develop treatment plans, and monitor healing progress across your patient panel.
• Document visits accurately and timely in the EMR.
• Work alongside a dedicated wound care technician who assists with supplies, setup, measurements, and charting support.
• Coordinate with facility nursing staff, primary care providers, and families.
• Travel to patient locations throughout the Sacramento region on a structured daily route.
