Job summary

Community Council Health Systems in Philadelphia is seeking a Mobile Therapist to deliver intensive therapeutic services to children, adolescents, and families in home, school, and community settings. You will develop treatment plans, provide individual and family therapy, and coordinate clinical services.

As lead clinician for assigned clients, you will supervise BHTs, review documentation, ensure timely reevaluations, and address clinical concerns in accordance with regulatory standards.

  • Complete timely reevaluations, authorizations, and plan revisions.
  • Maintain accurate clinical documentation in line with standards.
  • Review BHT documentation and provide supervision and guidance.
  • Collaborate with families, school personnel, and community agencies for coordinated care.
  • Respond to clinical emergencies and address ethical and safety concerns.
  • Participate in supervision and professional development while upholding policies.

Job description

The Mobile Therapist provides intensive therapeutic services to children, adolescents, and families in home, school, and community settings. The role includes developing treatment plans, delivering individual and family therapy, coordinating clinical services, clinical supervision of BHT's, and ensuring high-quality, timely documentation in accordance with IBHS and payer regulations.

Key Responsibilities
  • Develop, implement, and update individualized treatment plans based on evaluations, diagnoses, MDT recommendations, client progress, medical necessity, and payer requirements.
  • Provide individual and family therapy in approved settings while maintaining required intervention hours and ensuring services align with the treatment plan.
  • Serve as the lead clinician for assigned clients, coordinating care and monitoring treatment progress, barriers, and ongoing service needs.
  • Conduct treatment plan reviews, obtain required signatures, and complete timely reevaluations, reauthorizations, and plan revisions.
  • Complete, review, and maintain accurate clinical documentation, including treatment plans, progress notes, logs, and required reports, in accordance with regulatory and organizational standards.
  • Review and approve Behavioral Health Technician (BHT) documentation and provide clinical supervision, consultation, and guidance to support quality services.
  • Collaborate with families, school personnel, BHTs, Behavioral Consultants, community agencies, and other providers to ensure coordinated care and communicate relevant client updates.
  • Respond to clinical emergencies and promptly address and report safety, ethical, incident, and other urgent clinical concerns in accordance with established procedures.
  • Participate in required supervision, clinical meetings, trainings, and professional development while maintaining compliance with ethical standards, organizational policies, and assigned responsibilities.
