Job summary

Healthcare in Action is seeking a part-time Licensed Vocational Nurse to provide direct patient care in mobile street medicine to people experiencing homelessness. The role involves exams, vaccinations, wound care, and coordination with physicians and care teams in various community locations.

You will document care, develop care plans with patients and peers, and support health education while traveling between Pasadena, El Monte, and El Sereno.

Qualifications

• Associate's Degree/Technical degree in vocational nursing or higher.
• Current California Licensed Vocational Nurse is required.
• 2+ years of direct patient care experience in medical office, hospital, or field setting.
• Knowledge of ICD-10 coding preferred.
• Excellent assessment and communication skills; ability to stay calm in emergencies.

Responsibilities

• Conduct physical exams and assessments including medical histories, wellness exams, vaccinations, and blood draws if necessary.
• Provide wound care and patient monitoring in a mobile or street medicine setting.
• Maintain a safe, efficient environment and document patient visits accurately.
• Develop individualized care plans in collaboration with patients and physicians.
• Coordinate care with the clinical team and health plans; refer to resources as needed.
