Job summary

Healthcare in Action seeks a full-time Licensed Vocational Nurse to deliver street medicine via a mobile van and direct care in field settings for patients experiencing homelessness. The role involves travel between Union Stations in Pasadena, El Monte, and El Sereno, coordinating with a clinical team to assess and manage patient care.

Responsibilities include exams, vaccinations, wound care, education, and care planning in collaboration with physicians.

Qualifications

• Associate's Degree or higher in vocational nursing or related field.
• Current California Licensed Vocational Nurse is required.
• 2+ years of direct patient care in medical office, hospital, or field setting.
• ICD-10 Coding knowledge preferred.
• Excellent assessment skills; ability to problem-solve and prioritize changing patient conditions.
• Strong interpersonal skills to build relationships with providers and patients.
• Maintain patient confidentiality; knowledge of HIPAA requirements.
• Excellent verbal, written, and listening skills.
• Ability to work in a team and remain calm in emergencies.

Responsibilities

• Conduct physical exams and assessments; take histories; document health conditions; wellness exams; vaccinations; draw blood as needed.
• Engage with providers and patients to create a trusting environment; provide wound care and patient monitoring.
• Maintain a safe, efficient environment in conformance with standards.
• Develop individualized Care Plans in collaboration with patients and clinicians.
• Educate and advise on medical issues; follow company policies; handle lab specimens per policy.
• Ensure PCPs are aware of patient conditions and progress; report concerns as needed.
• Understand CMS and encounter data submission processes for the Health Plan.
• Assist in prevention, health promotion, and management of chronic conditions.
• Participate in QA, audits, and continuing education; provide referrals.
• Serve as a resource to providers and case management teams.
