Job summary

Healthcare in Action in California operates a street medicine program using a mobile van to deliver direct care to people experiencing homelessness. This part-time LVN role travels between Union Stations in Pasadena, El Monte, and El Sereno, providing vaccines, wound care, and coordination with the clinical team.

You will assess patients, document visits, support care plans, and maintain a safe clinical environment while collaborating with physicians and staff.

Qualifications

• Excellent assessment skills required and ability to problem-solve
• Strong interpersonal skills with patients and providers
• Knowledge of confidentiality/HIPAA requirements
• Ability to work in a team and maintain relationships with staff

Responsibilities

• Conduct physical exams and assessments, document health history and vaccinations
• Provide wound care and patient monitoring in a mobile/field setting
• Maintain a safe, efficient environment and follow standards
• Develop individualized Care Plans with patients and care team
• Coordinate with physicians, clinicians, and medical directors on patient care
• Educate patients and relay health information per policies
• Understand CMS data submission and encounter data processes
• Refer patients to appropriate medical resources as needed
• Serve as a resource to providers and case management teams
