Powerback Rehabilitation, LLC in Nashua, NH is seeking a part-time Speech Therapist for mobile outpatient therapy, serving Assisted Living/Independent Living and residential patients.

You’ll work with a multidisciplinary team, perform evaluations, develop plans of care, document findings, and ensure high-quality service in home, clinic, or facility settings.

Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to and from patients' residence.

Responsibilities

• Supervise CFYs, students, and support personnel per licensure and standards.
• Adhere to treatment schedule, document clinical and billing activities, and attend meetings.
• Participate in patient care conferences and related meetings.
• Refer to other disciplines to address patient needs.
• Collaborate with rehab team and physician to select appropriate therapy.
• Adapt plans of care to patient’s progress.
• Maintain safe environment and eliminate hazards.
• Order supplies and equipment as needed.
• Maintain and report on equipment condition and repair needs.
• Put patient service first with compassionate care.
• Screen and evaluate patients; develop goals and implement speech therapy plan.
• Coordinate adaptive communication devices and patient training.
• Identify hearing deficits and refer for further assessment.
• Select activities suited to patient capacity and interest.
• Instruct patients and evaluate progress and rehab potential.
• Reexamine as needed to progress plans of care.
• Coordinate home and community care with the team.
