Qualifications

• Must have a valid Speech Language Pathologist license in the state(s) of practice, or proof of license eligibility.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for older adults, in patients' homes or facilities.
• Supervise CFYs, students, and support personnel per state licensure and standards.
• Adhere to department and patient treatment schedules with timely documentation and meetings.
• Collaborate with rehabilitation team and physician to select appropriate therapy.
• Adapt plans of care based on patient needs and progress; ensure safe environment and equipment maintenance.
• Coordinate care in home and community settings with interdisciplinary team.
