Job summary

Cabarrus Rowan Community Health Centers seeks a Special Populations Coordinator to oversee services at mobile clinics, ensuring high-quality, cost-efficient, and culturally sensitive care compliant with policies. The SPC provides advocacy, health education, and support to groups including Migrant and Seasonal Farmworkers, Homeless, Veterans, and students, reducing barriers to health care access.

The SPC will supervise Special Population Patient Advocates, collaborate with the Community Health

Qualifications

Bachelor’s degree in public health, health education, communications or related field required or equivalent experience.
2 years of related work and management experience preferred.
Knowledge of health and human services infrastructure and computer skills; strong verbal and written communication required.
Bilingual in English and Spanish preferred.

Responsibilities

Optimize staffing, appointment schedules, workflows, and procedures with the Community Health Program Administrator.
Direct and coordinate services for all Special Population locations and supervise Special Population Patient Advocates.
Oversee patient registration and data entry into the EHR; ensure accuracy of demographic information.
Improve mobile unit performance and patient access measures; address patient grievances promptly.
