Job summary

County of Santa Clara seeks an Community Outreach Specialist to assist professional staff in developing and coordinating community service programs for designated populations. You will work in the community, collect data, prepare reports, and act as a liaison with community and school groups.

Responsibilities include outreach to identify needs, provide health education and paraprofessional counseling, refer to appropriate agencies, and support mobile health center operations.

Qualifications

• Training and Experience Note: knowledge and abilities are acquired through training and experience equivalent to completion of 60 semester college units.
• And One (1) year of experience providing education and para-professional counseling and outreach to specific communities and individuals, including collecting data, compiling reports, and serving as community and school liaison comparable to Mobile Outreach Driver in the County of Santa Clara.

Responsibilities

• Work with established community agencies and groups to identify unmet needs of designated populations and assist in planning services.
• Provide information to agencies and groups representing the community on programs, services, laws and regulations.
• Through outreach activities, locate designated populations and provide preventive education, referral and paraprofessional counseling.
• Refer individuals and groups to appropriate agencies as needed.
• Develop and maintain cooperative relationships with community agencies and groups and with department staff.
• Maintain contact with community members served and collect data to identify needs.
• Interpret community needs to departmental staff and assist with program planning.
• Assist in client recruitment and schedule/registration activities.
• May assist clients in completing health history and maintaining medical records for mobile health centers.
• May monitor vehicle safety checks and maintenance of mobile health center equipment.
