Job summary

Cabarrus Rowan Community Health Centers seeks a Special Populations Coordinator to oversee special population clinics on our mobile unit. You will ensure adherence to policies, coordinate patient advocacy, health education, and community outreach, and support access to care for migrant workers, homeless, veterans, students, and other groups.

Responsibilities

• Optimize staffing, appointment schedules, workflows, policies, and procedures with the Community Health Program Administrator.
• Direct, plan, supervise, and coordinate services for all Special Population locations.
• Oversee patient registration and accurate data entry into the EHR.
• Monitor mobile unit performance and ensure goals are met.
• Coordinate outreach activities and affiliated projects.
• Disseminate information to community partners about clinic services and public programs.
• Ensure excellent patient experiences and address complaints promptly.
• Communicate goals and procedures up and down the chain of command.
• Participate in staff meetings and community events to promote services and mission.

Summary of Position

The Special Populations Coordinator oversees all services provided at APHS’s special population clinics. The coordinator helps ensure that high-quality, cost-efficient, culturally sensitive services comply with all federal, state, and local agency policies and procedures, as well as APHS policies. This includes providing advocacy, health education, and support to special populations, including Migrant and Seasonal Farmworkers, Homeless and Public Housing Residents, Veterans, School-Based Health Students, and other special population groups, to help reduce social determinants of health and barriers to access to health care services.
The SPC collaborates with the clinical team to ensure efficient, effective daily operations and productivity across all APHS service lines. The Mobile Clinic Coordinator primarily oversees operational efficiency, including direct supervision of the Special Population Patient Advocates.
The Special Population Coordinator leads and facilitates key process improvements in patient access measures, registration clinic flow, population-based quality improvement, and patient experience across all service lines offered on the mobile unit. The SPC also serves as a liaison between community partners and the health center. This position requires flexible hours (including evenings) to support and reach our communities.

Minimum Qualifications

• Excellent oral and written communication skills. Ability to effectively develop and nurture relationships with a diverse group of stakeholders. Must be able to travel with a personal car.
• Experience: 2 years of related work and management experience preferred
• Additional skills required: Knowledge of health and human services infrastructure, moderate computer skills and excellent verbal and written communication skills.
• Additional skills preferred: Bilingual in English and Spanish.
• Education: Bachelor’s degree in public health, health education, communications or related program. An equivalent combination of education and experience will be considered.

Physical Requirements

Key Responsibilities

1. Works with the Community Health Program Administrator to optimize staffing, appointment schedules, workflows, policies, and procedures as needed to meet goals.
2. Directs, plans, organizes, supervises, and coordinates services for all Special Population locations.
3. Responsible for patient registration, including accurate collection of demographic information and data entry into the EHR.
4. Monitors and ensures that mobile unit operational performance goals are maintained.
5. Assists in coordinating outreach activities, including affiliated projects.
6. Disseminates information to all community partners regarding clinic services and state and local public services.
7. Ensures excellent patient experience and that patient complaints and grievances are addressed and resolved in a timely, responsive manner.
8. Ensures communication of goals, objectives, policies, and procedures up and down the chain of command through meetings, huddles, and other forms of communication.
9. Participate in health center/clinic staff meetings to provide input.
10. Develops, discovers, and attends community events to promote coverage options and the organization's mission and services.
11. Represents the company professionally and positively to enhance and promote the organization's core values and mission.
12. Works independently, exercises independent judgment in problem-solving, and determines appropriate actions during emergency/critical situations.
13. Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving, and/or controlling program goals, objectives, and services.
