Job summary

Safe Life Home Health Care, LLC is seeking an office-based Clinical Case Manager to support referral intake, case triage, coordination of care, and communication with patients, families, referral sources, and the clinical team. This RN role is ideal for someone organized, responsive, and able to manage a high volume of incoming cases in a fast-paced home health setting.

• Active RN license in good standing.
• Experience in home health, intake, referral management, case coordination, or clinical triage preferred.
• Strong understanding of home health services, documentation, and care coordination processes.
• Excellent communication, organizational, and multitasking skills.

Responsibilities

• Provides oversight of all patient care services and personnel, including assignments, coordination of patient care, referrals, and development of individualized patient plans.
• Serve as first line for resolution of staff concerns and issues.
• Plan and participate in delivery of home health services, coordinating with providers, vendors, and facilities.
• Coordinate with hospitals, physicians, discharge planners, visiting clinicians, patients and internal teams.
