Job summary

Summit Home Care & Hospice is seeking an experienced Physical Therapy Assistant (PTA) to join our team in Lawrence, IN. You will work directly with patients in their homes, guiding therapeutic exercises to restore mobility and strength, while using modalities like electrical stimulation and ultrasound to reduce pain and swelling.

The role requires travel to patients’ homes, holds a $68,000–$78,000 annual pay under a PPV structure plus a $5k sign-on bonus paid separately.

Qualifications

• Active PTA license in Indiana.
• Associate Degree in Science or Applied Science from an accredited institution.
• Willingness to travel to patients' homes to provide care.
• Previous experience in physical therapy or rehabilitation preferred.
• Excellent verbal and written communication and organizational skills.
• Ability to build rapport with in-home patients and ALF/ILF staff.

Responsibilities

• Work directly with patients on therapeutic exercises to regain mobility and strength.
• Administer modalities such as electrical stimulation and ultrasound to reduce pain and swelling.
• Collaborate with PT and care team to achieve well-rounded care.
• Develop and implement PT programs focused on rehabilitation.
• Evaluate therapy effects and communicate patient progress to the PT.
• Observe and reassess progress and adjust the plan of care as needed.
• Share information with the PT when adjustments are needed.
• Provide personalized care for each patient.
• Provide training for using assistive devices (crutches, canes, prosthetics).
• Record and document patient care and progress.
