Job summary

Summit Home Care & Hospice is seeking an experienced Physical Therapy Assistant (PTA) to join our team in Lawrence, IN. You will work directly with patients in their homes, guiding therapeutic exercises to restore mobility and strength, while using modalities like electrical stimulation and ultrasound to reduce pain and swelling.

The role requires travel to patients’ homes, holds a $68,000–$78,000 annual pay under a PPV structure plus a $5k sign-on bonus paid separately.

Qualifications

• Active PTA license in Indiana.
• Associate Degree in Science or Applied Science from an accredited institution.
• Willingness to travel to patients' homes to provide care.
• Previous experience in physical therapy or rehabilitation preferred.
• Excellent verbal and written communication and organizational skills.
• Ability to build rapport with in-home patients and ALF/ILF staff.

Responsibilities

• Work directly with patients on therapeutic exercises to regain mobility and strength.
• Administer modalities such as electrical stimulation and ultrasound to reduce pain and swelling.
• Collaborate with PT and care team to achieve well-rounded care.
• Develop and implement PT programs focused on rehabilitation.
• Evaluate therapy effects and communicate patient progress to the PT.
• Observe and reassess progress and adjust the plan of care as needed.
• Share information with the PT when adjustments are needed.
• Provide personalized care for each patient.
• Provide training for using assistive devices (crutches, canes, prosthetics).
• Record and document patient care and progress.

At Summit, we believe healthcare should be personal, compassionate, and delivered where it matters most - at home.Physical Therapy Assistant (PTA):Hours: Monday - Friday; Standard Business HoursWork Setting: On the Road for Patient Visits & Training at the Local Office or RemotelyTravel: Required + Mileage ReimbursedPay Structure: $68,000-$78,000 on a PPV structure (Ability to make more with a caseload larger than 30 visits per week) + $5k sign on bonus paid in separate installmentsLocation: Lawrence, IN (East Side of Indiana)Responsibilities:Work directly with patients on therapeutic exercises designed to help regain full mobility, strengthen their muscles, and improve coordination.Administer various physical modalities, such as soft tissue massage, electrical muscle stimulation and ultrasounds, to help relieve pain, improve circulation, and decrease swelling in the muscles.Collaborate with other team personnel to achieve well-rounded care.Develop and implement physical therapy programs that focus on rehabilitation.Evaluate effects of therapy treatment and communicate patients' progress.Observe and reassess a patient’s progress so they can modify the plan of care as needed.Share necessary information with the Physical Therapist (PT) if/when adjustments need to be made.Ensure that their patients receive personalized care.Provide training for using assistive devices, such as crutches, canes, or prosthetics.Record and document patient care services and progress.Keep the Physical Therapist (PT) up to date on the patient’s progress and ensure they know which aspects of the treatment are working or need adjustments to help determine the best path forward.Qualifications:Associate Degree in Science or Applied Science from an Accredited Institution.Active PTA License in the state of Indiana.Must be willing to travel to patients’ homes to provide care.Previous experience in physical therapy, rehabilitation, or other areas preferred.Excellent verbal and written communication and organizational skills.Ability to build rapport with in-home patients, Physical Therapist (PT) and Assisted Living Facility (ALF)/Independent Living Facility (ILF staff), as necessary.Work in a fast-paced, flexible work environment.Must be able to handle the physical workload with kindness, compassion, and empathy.Strong leadership qualities.Why Summit?We offer a well-rounded benefits package to support you both at work and at home.
