Summit Home Care & Hospice in Lawrence, IN seeks a Physical Therapy Assistant to support home visits and remote training. The PTA will collaborate with Physical Therapists and care teams to develop and adjust rehab plans, monitor progress, and promote mobility and independence for patients.

Qualifications

• Associate degree in science or applied science from an accredited institution.
• Active PTA license in Indiana.
• Willingness to travel to patients' homes.
• Previous PT/rehabilitation experience preferred.
• Excellent verbal and written communication and organizational skills.

Responsibilities

• Work with patients on therapeutic exercises to regain mobility and strength.
• Administer modalities (massage, e-stim, ultrasound) to relieve pain and swelling.
• Collaborate with team to deliver comprehensive care.
• Develop and implement rehabilitation programs and reassess progress.
• Document patient care and progress; relay updates to PT.
• Provide training on assistive devices and ensure personalized care.
• Maintain progress records and adapt plans as needed.
