Job summary

Summit Home Care & Hospice is seeking an experienced Physical Therapist (PT) to join our team in Tipton, IN. This PRN role covers patient visits on the road and potentially remote training at the local office.

Responsibilities include evaluating patients, developing rehab plans, educating families, and coordinating with physicians. Indiana PT licensure and 2+ years of PT experience preferred; home health experience is a plus.

Qualifications

• Bachelor’s, master’s or doctorate in physical therapy from an accredited program.
• Active license to practice as a physical therapist in the state of Indiana.
• 2 years of previous clinical experience as a physical therapist, preferred.
• Prior home health or community-based experience is preferred but not required.
• Strong verbal and written communication skills, excellent organizational abilities, and the capacity to work independently and as part of a team.

Responsibilities

• Deliver physical therapy services per physician’s plan of care.
• Assess functional status and mobility needs of patients.
• Develop and implement rehab programs focused on improvement.
• Evaluate home environments for safety and accessibility.
• Educate patients and caregivers on therapies and devices.
• Collaborate with interdisciplinary team to ensure patient-centered care.
• Prepare clinical documentation and discharge plans per policy.
