Job summary

Powerback is seeking a Physical Therapist Assistant to provide outpatient rehabilitation in in-home settings for older adults. The role supports care across Vernon, Ellington, Tolland, South Windsor, CT, with a flexible schedule in various home environments.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to and from patients' residences.

Responsibilities

• Attends patient care conferences and meetings as a representative of physical therapy.
• Adheres to department and patient treatment schedule, and completes timely clinical and billing documentation.
• Works with the rehabilitation team to implement programs and activities for each patient.
• Acts as an assistant to the Physical Therapist during tests, evaluations and procedures.
• Instructs patients, families and caregivers in the skills and techniques of the treatment program under supervision.
• Maintains accurate and timely documentation for all patients treated.
• Cleans, maintains, identifies, and reports equipment issues needing repair.
• Puts patient service first with compassionate, patient-centered care.
• Implements individualized treatment plans established by the Physical Therapist.
• Provides treatments including therapeutic exercise, gait training, manual therapy, airway clearance techniques and more.
