Job summary

Powerback Rehabilitation seeks a Physical Therapist to provide outpatient rehabilitation in the home setting for older adults in Salem, NH and surrounding areas. You will evaluate, design, and deliver therapy programs to promote independence and safety in the patient’s home.

Responsibilities include supervising therapy staff, timely documentation, and collaborating with the rehab team to address comprehensive patient needs. A $5,000 sign-on bonus is offered for full-time hires.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Master’s degree in Physical Therapy or Master’s in PT-related field with a Bachelor’s in Physical Therapy.
• Bachelor’s in Physical Therapy plus 5 years of PT experience.

Responsibilities

• Provide outpatient rehabilitation in the patient’s home or in an independent living facility.
• Develop and deliver individualized therapy plans to promote independence and safety.
• Supervise PT assistants, temporary licensees, students, and support staff as required.
• Ensure timely clinical and billing documentation and attend care conferences.
• Refer to other disciplines to address comprehensive patient needs.
• Adapt treatment plans based on patient progress and capabilities.
