Qualifications

• Driver’s license and reliable transportation to patient homes.
• Experience or readiness for in-home outpatient rehab is preferred.

Responsibilities

• Provide outpatient rehabilitation services in in-home or facility settings for older adults.
• Assess, design, and implement PT plans of care and document outcomes.
• Educate patients and caregivers; coordinate with the rehab team and refer to other disciplines as needed.
• Supervise PT Assistants and students as required; ensure safety and compliance.
