Talin Home Health is seeking a dedicated Physical Therapist to evaluate and treat patients in their homes, collaborating with medical professionals to create individualized care plans.

The ideal candidate holds a DPT from an accredited program, has clinical experience (orthopedics or home health), and demonstrates strong knowledge of anatomy, medical terminology, and discharge planning. Excellent interpersonal skills are essential.

Qualifications

• Degree in Physical Therapy from an accredited institution is required.
• Proven experience in a clinical setting preferred, especially orthopedics or home community care.
• Strong knowledge of medical terminology and anatomy is essential for communication and planning.
• Familiarity with discharge planning processes and systems used in patient care is advantageous.
• Excellent interpersonal skills to build rapport with patients and collaborate with healthcare teams.

Responsibilities

• Conduct thorough evaluations of patients’ physical conditions and functional abilities.
• Develop individualized care plans that address specific patient needs.
• Implement treatment programs using exercises, manual therapy, and modalities.
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes.
• Educate patients and families on rehabilitation, home exercises, and injury prevention.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care, including discharge planning.
• Maintain accurate documentation of evaluations, treatments, and progress per clinic policies.
