Talin Home Health in Virginia is seeking a dedicated Physical Therapist to evaluate and treat patients in their homes, improving function and quality of life. You will work with clients and healthcare professionals to develop tailored care plans and ensure safe, effective therapy in the home setting.

The role requires a degree in Physical Therapy, clinical experience (orthopedics or home care preferred), strong knowledge of anatomy and medical terminology, and excellent communication to educate

Qualifications

• A degree in Physical Therapy from an accredited institution is required.
• Proven experience in a clinical setting is preferred, particularly in orthopedics or home community care.
• Strong knowledge of medical terminology and anatomy is essential for effective communication and treatment planning.
• Familiarity with discharge planning processes and systems used in patient care will be advantageous.
• Excellent interpersonal skills to build rapport with patients and collaborate with healthcare teams.

Responsibilities

• Conduct thorough evaluations of patients’ physical conditions and functional abilities.
• Develop individualized care plans that address the specific needs of each patient.
• Implement treatment programs utilizing various therapeutic techniques, including exercises, manual therapy, and modalities.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Educate patients and their families on the rehabilitation process, home exercises, and injury prevention strategies.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care, including discharge planning for inpatient cases.
• Maintain accurate documentation of patient evaluations, treatments, and progress in accordance with clinic policies.
