Job summary

Nurturing Home Healthcare in Houston, TX seeks a dedicated Physical Therapist (PT) for home health on a PRN basis. You will evaluate patients at home, develop plans, and deliver therapeutic interventions to improve mobility and safety, collaborating with a multidisciplinary team.

The ideal candidate has a TX PT license, at least 1 year of PT experience, and strong communication skills to educate patients and families. CPR is required or obtainable on hire.

Qualifications

• Texas PT license in good standing.
• Graduate of an accredited Physical Therapy program.
• Minimum of 1 year PT experience; home health preferred.
• Current CPR certification or ability to obtain.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive PT evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders.
• Provide skilled interventions to improve strength, mobility, balance, endurance, and independence.
• Monitor progress toward established goals and adjust plans as needed.
• Educate patients and caregivers on plans, exercises, safety and prevention.
• Collaborate with physicians, nurses, therapists, and other staff to coordinate care.
• Train patients on assistive devices and home exercise programs.
• Complete timely documentation in electronic systems.
• Participate in case conferences and interdisciplinary meetings.
• Identify changes in patient condition and escalate appropriately.
