Job summary

Doctor's Choice Home Care & Hospice Texas is seeking a licensed Physical Therapist to provide in-home physical therapy in Texas. The role involves following physician orders, developing and evaluating care plans with patients and caregivers, and educating families.

Responsibilities include supervising PTAs and HHAs, communicating with physicians, and contributing to QAPI and interdisciplinary assessments. Requires state licensure, CPR, and 100% travel.

Qualifications

• Licensed physical therapist in the state(s) of practice.
• Two years of home health experience preferred.
• CPR certification (American Heart Association or American Red Cross).
• Excellent written and verbal communication skills.
• Must have a valid driver's license, auto insurance and reliable transportation.
• 100% travel required.

Responsibilities

• Administers physical therapy per physician orders and plan of care in patients' homes.
• Develops and evaluates the plan of care with the patient, representative (if any), and caregiver.
• Participates in patient, caregiver, and family counseling and caregiver education.
• Supervises Physical Therapy Assistants and Home Health Aides as needed.
• Communicates with physicians and other health care practitioners about the plan of care.
• Contributes to QAPI program and ongoing interdisciplinary assessments.
