Job summary

Enhabit Home Health & Hospice seeks a licensed Physical Therapist to examine and treat patients at home, improving mobility and daily living activities. You will provide skilled care per physician orders, educate families, and collaborate with the care team under the Branch Director’s supervision.

The role requires a valid state licensure and the ability to drive in all weather, with travel as needed to patient locations.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.

Responsibilities

• Examine and treat patients to improve mobility and function.
• Administer skilled care under physician orders and plan of care.
• Educate family and health care team members on care plans.
• Work under direction of Branch Director and coordinate with the team.
