Job summary

Enhabit Home Health & Hospice is seeking a Physical Therapist to examine and treat patients with physical impairments, focusing on mobility and daily living activities. Services are delivered under physician orders and a plan of care, with supervision from the Branch Director.

The position requires valid licensure in the state, a driver's license, insurance, and reliable transportation to service patients in diverse weather conditions.

Qualifications

• Must possess a valid state driver's license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Examine and treat patients with physical impairments using physical modalities.
• Improve mobility, self-care, and daily living activities under plan of care.
• Provide skilled care and educate family and health care team under supervision of Branch Director.
