Qualifications

Responsibilities

• Perform initial and ongoing physical therapy evaluations in the community or patient residence.
• Implement treatment plans on a schedule and adjust as patient progress.
• Provide therapy directly or through supervision and teaching of others.
• Evaluate mobility, strength, and function; set realistic, achievable goals.
• Contribute to quality improvement initiatives and ongoing education.
