Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to join our growing team. This role delivers in-home physical therapy with a flexible schedule within our service area, supporting patients and families in their homes.

The position requires an active California PT license, graduation from an accredited PT program, and the ability to travel to patient homes. We offer competitive salary and excellent benefits in a supportive environment.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform SOC evaluations, ROC, Recert, and DC.
• Assess patient needs, plan interventions, evaluate outcomes, and modify care to meet PT goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on PT programs and home exercise regimens.
