Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to join our growing home-health team. You will deliver skilled physical therapy services in patients' homes, align with physician orders, and support families with personalized care plans.

The ideal candidate holds an active California PT license, excels in documentation, communication, and teamwork, and is willing to travel within our service area.

Qualifications

• Active California Physical Therapy license.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations within our service area.

Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions per physician orders.
• Perform SOC, ROC, recertifications, and discharges.
• Assess patient needs, plan interventions, evaluate outcomes, and modify care.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on PT programs and home exercise regimens.
