Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to deliver home-based physical therapy services with individualized care.

You will perform SOC/ROC/Recert/DC evaluations, educate families, travel to patient homes across our service area, and collaborate with an interdisciplinary team to achieve patient goals. The role offers flexible scheduling, professional development, and a supportive team environment.

Qualifications

• Licensed physical therapist in California with a degree from an accredited program.
• Strong documentation and organizational skills are required.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes within service area.

Responsibilities

• Provide comprehensive physical therapy services per physician orders.
• Conduct SOC, ROC, recertifications, and discharges.
• Assess patient needs, plan interventions, and evaluate outcomes.
• Collaborate with the interdisciplinary team on patient goals.
• Educate patients, families, and caregivers on therapy programs and home exercises.
