Job summary

Qualifications

  • BA or AS in related field or High School diploma and 2 years’ experience in related field.
  • Fluency in American Sign Language (ASL) and written English is required.
  • ASL proficiency preferred with understanding of culturally diverse DHH populations.
  • Ability to thrive in a fast-paced, team-oriented environment and work independently.
  • Strong organizational skills and ability to multi-task.
  • Must hold a valid driver's license and have access to an insured vehicle for transportation.

Responsibilities

  • Deliver rehabilitative services and coordinate care to meet person-centered goals.
  • Participate in treatment planning and meetings as required.
  • Provide transportation to clients as connected to treatment needs.
  • Document progress notes, treatment plans, and changes in daily records.
  • Maintain clinical records meeting Rehab Option standards and billing codes.
  • Demonstrate proactive communication with staff and clients to support effective relationships.
  • Assist with transitions between group homes and independent living, including logistics of moves.
  • Ensure paperwork, reports, trainings, and certifications are up to date.
  • Provide crisis intervention and access emergency services when needed.
  • Identify community integration barriers for persons served and address them.
  • Participate in or facilitate group work as designated by supervisor.
  • Attend supervision and trainings and maintain required certifications (CPR/First Aid, Crisis Management, MAP).
  • Maintain professional, courteous demeanor at all times.
