Job summary

COMMUNITY SUPPORT SERVICES For the DEAF, Inc. is seeking a Personal Supports Case Manager to provide direct support to Deaf and Deaf-Blind adults in the Baltimore metro area.

The role is in person and offers both FT and PT hours, with ASL fluency required and a valid MD driver’s license. Applicants should have a Bachelor’s degree or a HS diploma with 3 years of relevant experience, excellent time management, and a patient, respectful, team-oriented approach.

Qualifications

• Bachelor’s degree or HS diploma with 3 years relevant experience.
• Valid MD Driver’s License with 2 points or less.
• Fluent in American Sign Language (ASL).

Responsibilities

• Directly support Deaf or Deaf-Blind Adults with Intellectual/Developmental Disabilities with daily living activities and personal cares.
• Provide home-based assistance including cleaning, laundry, and cooking as needed.
• Help with transportation to medical appointments, shopping, and social events, and assist with mail.
• Contribute to the PCP plan and implement activities as identified in meetings.
