Job summary

Powerback Rehabilitation seeks a Physical Therapist to deliver outpatient rehabilitation in in-home settings for the elderly population, including private homes and independent living facilities. You will assess needs, design individualized PT programs and coordinate care with the rehab team.

You will educate patients and caregivers, promote independence, supervise support staff as needed, and ensure documentation and safety standards are met.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility.
• They must have a Master’s degree in Physical Therapy; or
• They must have a Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or
• They must have a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
• The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
• Must have a valid driver’s license or reliable, independent transportation to and from the patients’ residence.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for older adults.
• Evaluate and treat in the patient’s home or in an independent living facility.
• Develop and deliver physical therapy programs to facilitate rehabilitation.
• Identify need for and make referrals to other disciplines.
• Instruct, educate and train patients and caregivers to promote independence.
• Direct activities to promote and maintain health.
• Supervise PT assistants, temporary licensees, students and support personnel as required.
• Maintain timely clinical and billing documentation.

Education

Master’s degree in Physical Therapy

Bachelor’s degree in Physical Therapy

5 years of PT experience

License in state(s) of practice or license eligibility

US-equivalent evaluation for foreign PT degree
