Job summary

Powerback is seeking a Physical Therapist to deliver outpatient rehabilitation in home-based and facility settings for older adults. The role emphasizes individualized plans, patient education, and collaboration with the rehabilitation team.

Responsibilities include evaluation, treatment, documentation, and supervision of assistants while maintaining safety and quality care. A valid PT license or eligibility is required; a master’s degree is preferred, with licensure requirements per state.

Qualifications

• Must hold a valid Physical Therapy license in the practicing state or be eligible for licensure.
• Master's degree in Physical Therapy is required; a related master’s with a PT bachelor’s is acceptable.
• At least 5 years of progressively responsible PT experience is preferred; foreign degrees must be evaluated for U.S. equivalence.

Responsibilities

• Provide in-home/outpatient physical therapy in the older adult population.
• Assess, develop, and deliver individualized therapy programs; refer to other disciplines as needed.
• Educate patients and caregivers; document and manage care plans and progress.
• Supervise PT assistants and students per licensure rules.
• Promote safety, order supplies, and participate in care conferences.
