Job summary

Prosperity Placement Solutions seeks a Mobile Nurse Practitioner to provide home-based, field-based patient care across the San Fernando Valley and Antelope Valley, CA. You will deliver acute, post-acute, and transitional care with clinical autonomy, collaborating with an interdisciplinary team.

You will care for medically complex and recently discharged patients, perform home visits, and develop individualized care plans while coordinating with physicians and other providers to improve outcomes.

Qualifications

• MSN required.
• Adult/Geriatric NP certification.
• Active RN license.
• Active CA NP license.
• Minimum 1 year NP clinical experience.
• Strong assessment and care management skills.
• Excellent communication and interpersonal skills.
• Ability to work independently with a multidisciplinary team.
• Valid driver's license and reliable transportation.
• Current TB test or willingness to obtain prior to employment.

Responsibilities

• Provide acute, post-acute, and transitional care directly in patients' homes.
• Evaluate medically complex and recently discharged patients.
• Perform comprehensive physical assessments.
• Diagnose and treat acute and chronic conditions.
• Develop individualized treatment and care plans.
• Manage medications and follow-up care.
• Identify complications and intervene early to prevent hospitalizations.
• Educate patients and families on disease management and prevention.
• Coordinate care with physicians, nurses, pharmacists, social workers, and other providers.
