Qualifications

• Active and unrestricted California Nurse Practitioner license.
• Active DEA license.
• Current BLS certification.
• Strong clinical assessment and diagnostic skills.
• Ability to practice independently while collaborating with an interdisciplinary care team.
• Commitment to patient-centered, preventive, and value-based care.
• Excellent communication and relationship-building skills.
• Comfortable providing care in home, community, and virtual settings.

Responsibilities

• Provide mobile nurse practitioner services through home visits, community-based care, and virtual appointments.
• Work within a value-based care model focused on proactive interventions and quality outcomes.
• Collaborate with an interdisciplinary team to coordinate patient care and ensure seamless service delivery.
• Promote preventive care and patient education during visits across settings.

Practice beyond the walls ofa traditional clinic as a full-time Mobile Nurse Practitioner serving patientsthroughout San Gabriel Valley and East Los Angeles, California. In this role,you will bring high-quality, relationship-based care directly to patientsthrough home visits, community-based care, and virtual appointments whileworking within a patient-centered, value-based care organization focused onimproving outcomes and supporting long-term health.

Our care model is focused onquality outcomes, preventive care, proactive interventions, and coordinatedpatient support rather than high visit volume. You will enjoy a high level ofclinical autonomy while working alongside an interdisciplinary team dedicatedto helping patients achieve better health outcomes.

Position Highlights

• Full-Time Schedule:Monday–Friday | 8:00 AM–5:00 PM
