Steele Healthcare Solutions, LLC is seeking a Mobile Nurse Practitioner to deliver comprehensive primary care in patients’ homes and via virtual visits across the San Fernando Valley and Antelope Valley.

Ideal candidates will value autonomy, relationship-based care, and practicing at the top of their license. Network with an interdisciplinary team focused on improving outcomes for vulnerable patients.

Qualifications

• MSN from an accredited program and current NP certification.
• At least 1 year of NP or equivalent advanced practice experience.
• Active, unrestricted California RN and NP licenses.

Responsibilities

• Provide comprehensive primary care to medically complex patients in patients' homes and through virtual visits.
• Collaborate with an interdisciplinary team to improve outcomes for vulnerable and underserved patients.
• Travel throughout the San Fernando Valley and the Antelope Valley to deliver care.

Education
