This nationally recognized healthcare organization is seeking a Mobile Nurse Practitioner to provide acute, post-acute, and transitional care directly in patients' homes throughout the San Gabriel Valley, California.

The role offers significant clinical autonomy, collaboration with an interdisciplinary care team, and a strong emphasis on reducing hospitalizations while delivering compassionate, patient-centered care.

Qualifications

• MSN required and active NP certification.
• Active RN license and CA NP license.
• Minimum 1 year NP clinical experience.
• Strong assessment, diagnostic and care management skills.
• Valid driver's license and reliable transportation.
• TB test current or willingness to obtain.

Responsibilities

• Provide acute, post-acute, and transitional care in patients' homes.
• Evaluate medically complex and recently discharged patients.
• Perform comprehensive physical assessments and diagnose conditions.
• Develop individualized treatment plans and manage medications.
• Coordinate follow-up care and communicate with the care team.
• Maintain timely clinical documentation and ensure safety.
