Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) is seeking a compassionate Mobile Adult/Geriatric Nurse Practitioner to deliver acute, post-acute, and transitional care to medically complex and recently discharged patients throughout the San Fernando Valley and Antelope Valley in California.

The role offers clinical autonomy within an interdisciplinary care team, focusing on improving patient outcomes, reducing hospital readmissions, and supporting safe recovery at home.

Qualifications

• MSN required.
• Adult/Geriatric NP certification.
• Active, unrestricted CA RN license.
• Active, unrestricted CA NP license.
• Minimum 1 year NP clinical experience.
• Strong assessment, diagnostic, and care management skills.
• Valid driver’s license and reliable transportation.
• Ability to work independently with a multidisciplinary team.
• Current TB test or willingness to obtain before employment.

Responsibilities

• Provide acute, post-acute, and transitional care to medically complex and recently discharged patients.
• Deliver field-based, autonomous clinical care with interdisciplinary team collaboration.
• Support patient outcomes and reduce readmissions through home-based services.
