Your Health Organization in Georgia is hiring a Visiting Nurse (RN/LPN) to support patients in the Savannah area. The role requires home visits, care coordination, and telemedicine initiation, traveling to patients' homes and facilities.

You will work 8-hour days, Monday through Friday, with a focus on quality, compliant care. Qualified candidates are licensed nurses with 3+ years of clinical experience, strong communication, and teamwork abilities.

Qualifications

• Must be a licensed nurse (RN preferred).
• Minimum of three (3) years clinical experience.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal skills.

Responsabilités

• Facilitate receiving all medical records from patient’s providers.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if appropriate.
• Capture diagnoses and ensure accuracy.
• Complete AWVs for provider review.
• Conduct cognitive impairment screenings and SDoH assessments.
• Document Care Management activities and keep plans up to date.
• Coordinate care and communicate with care team.
• On-call may be required.
