Job summary

A health care organization is looking for a field-based Nurse Practitioner (AGNP or CNP) in the Greater Twin Cities Metro Area. The role offers an independent and flexible environment with responsibilities including visiting senior communities for primary care. Applicants need an active Minnesota NP license and preferably AGNP certification. Competitive compensation between $105,000 - $120,000 annually, with full-time or part-time options. Wound care certification costs covered for candidates not certified.

Qualifications

• Must have an active Certified Nurse Practitioner license in Minnesota.
• AGNP certification is preferred for applicants.
• Experience in geriatrics, family practice, or long-term care is desired.

Responsibilities

• Visit assisted living, memory care, and senior communities to provide primary care.
• Address wounds, chronic conditions, and preventative care in place.
• Partner with care teams and families to ensure holistic support.
