Job summary

Ennoble Care in Camden, NJ seeks a Nurse Practitioner or Physician Assistant to deliver primary and palliative home-based care to homebound patients in the Camden County region. Under physician supervision, you will perform exams, create care plans, order tests, and coordinate with staff to ensure high-quality, patient-centered care.

Qualifications

• Board Certified NP or PA in New Jersey.
• Strong clinical skills with the ability to treat, assess, and diagnose confidently.
• Positive demeanor and strong people skills.
• Ability to problem solve and operate independently.
• 1–2 years of primary care experience with geriatric patients (hospice experience preferred).

Responsibilities

• Provide high quality care to primary and palliative patients in and around Camden County, NJ.
• Assess health through physical examinations and medical histories; create care plans.
• Communicate effectively with office staff and clinical operations.
• Provide patient-centered care to a panel of patients.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients, families and caregivers during visits.
• Order diagnostic testing as needed.
• Prescribe medications and appropriate interventions.
