Ennoble Care in Cumberland County, NJ is seeking a Nurse Practitioner to provide primary and palliative care to homebound patients. You will partner with a physician supervisor, perform comprehensive exams, and create individualized care plans for a diverse geriatric population.

In this full-time role, you will order diagnostics as needed, coordinate with families, and deliver compassionate, patient-centered care in home settings.

Qualificações

• Board-certified NP or PA with New Jersey license.
• Strong clinical and diagnostic skills required.
• Independent worker with positive demeanor and strong people skills.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsabilidades

• Provide primary and palliative care to homebound patients in Cumberland County, NJ.
• Perform physical examinations and update medical histories to create care plans.
• Communicate effectively with office staff and clinical operations across platforms.
• Attend quarterly medical staff meetings and coordinate with families and caregivers.
• Develop impressions, diagnoses, and treatment plans; order necessary diagnostics.
