Job summary

Avail Health Medical Practice is seeking a Mobile Nurse Practitioner to provide patient-centered care to I-SNP members in skilled nursing facilities. This part-time, W-2 role spans two days per week with flexible weekend availability and remote work options supported by a HIPAA-compliant workspace.

Ideal candidates hold MSN or DNP, active WV NP license, and NP certification (FNP/AGNP) with 1–3 years in geriatrics or long-term care. Travel within Keyser, WV region is required.

Qualifications

MSN or DNP from an accredited program.
Current national NP certification (FNP or AGNP).
Active WV NP license and WV DEA registration (or willing).
1–3 years clinical experience in geriatrics or long-term care preferred.

Responsibilities

Deliver primary and acute care to I-SNP members in SNFs.
Assess and manage acute illnesses and geriatric conditions.
Develop and adjust individualized care plans with the team.
