Job summary

Avail Health is seeking a Full-Time Mobile Nurse Practitioner to serve I-SNP members in skilled nursing facilities across northern West Virginia. The role requires residence in WV and travel within the region; it is hybrid with a normal schedule of Monday–Friday 8:00 a.m.

to 5:00 p.m. You will deliver comprehensive primary and acute care, perform HRAs, and coordinate with the interdisciplinary team to support ongoing patient needs while maintaining compliant EMR documentation and regulatory

Qualifications

• Active, unrestricted West Virginia NP license and WV RN license; WV DEA registration.
• 1–3 years NP experience, ideally geriatrics, long-term care, mobile, or value-based care.
• Familiarity with risk adjustment, HEDIS/STARs, and care transitions preferred.
• Must be able to travel within assigned region and work independently in a field-based setting.

Responsibilities

• Deliver comprehensive primary and acute care to I-SNP members in SNFs.
• Assess, diagnose, and manage acute and geriatric conditions in a medically complex population.
• Conduct HRAs and develop individualized, goal-oriented care plans with the care team.
• Participate in weekly IDT meetings to review needs and improve outcomes.
• Coordinate transitions of care and monitor patient status to prevent complications.
• Maintain timely patient documentation in the EMR and comply with program requirements.
